Affordable nursing at the doorstep is the new fad for many in urban India.
When S.P. Kamra (75) returned home from hospital after treatment for a stroke, his family members were worried. Completely bed-ridden in his Delhi home, Kamra needed constant and expe­rt care. As his son, Praveen, looked for optio­ns, someone suggested home care services. From monitoring Kamra’s ventilator parameters, to maintaining an input-output chart for fluid balance, chest and limb physiotherapy, to changing his body position every hour, even providing psychological support, the expert attendants from the private service provider, Health Care at Home, did everything to nurse him back to health. And his son, Praveen, could not have been happier. “With the support and the treatment, my father’s health status has improved…he has been weaned off the ventilator and his vitals are within the normal level. His neurological conditions have improved too. With physiotherapy, his lower limbs have gained back strength,” Praveen tells Outlook. “Psychological support and counselling by the attendants (also) helped him get well faster”.

Hundreds of miles to the west, in Mumbai, the family of K. Jayakantham also faced a similar dile­mma. The 82-year-old woman was suffering from severe pneumonia besides advanced deme­ntia. Her son, K Sampath, then turned to Zoctr, another home healthcare service. “Initially, we were taking care of our mother. But later we realised that we need trained help. Zoctr provided us with two trained nurses, an ayah and a male attendant,” says Sampath. “We are very happy with their services. They know all the medical procedures and they check the parameters regularly. Most importantly they are very helpful.”

Once the domain of the neighbourhood ayaah or the even the untrained domestic help, post hospitalisation care at home is now provided by professional experts in urban areas of India, where the gap between the number of patients and hospital beds is huge. Data from the National Health Profile 2017 show that there is just one government hospital bed for every 2,046 people. One research shows that India shares more than 20 per cent of the global disease burden, it accounts for only eight per cent of doctors and six per cent of hospital beds. Private hospitals work on the ALOS (average length of stay) model—the increased availability of hospital beds translates to more surgeries and hence higher revenue. Hospitals prefer to discharge patients who are stable and use the infrastructure for surgeries and other medical procedures for critical patients.

Experts say the demand for home healthcare services is growing exponentially due to several reasons including high hospitalisation costs, rising elderly population, need for personalised medical attention, dismantling of traditional joint family, lack of quality healthcare service network and increase of non-communicable diseases that require care, but not necessarily hospitalisation. According to an estimate by the University of Pennsylvania, the home healthcare industry in India is estimated to grow from Rs 7,800 crore in 2018 to Rs 91,000 crore by 2025.

The rising number of senior citizens—about 150 million—is also fuelling the industry. In a country with an increased movement of people globally for work, elderly parents or family members with chronic diseases are often left behind, and alone. And approximately 29.7 per cent of the elderly population suffers from depre­ssion and this is mainly because of loneliness. Swadeep Srivastava, managing partner of IVH SeniorCare says, “Loneliness and depression among the elderly population is our main concern...What patients of depression require is company, someone with whom they can enjoy some quality time. If they are living alone, their mental condition deteriorates even faster. The care managers talk to them and keep them eng­aged. They are more like buddies. They take the elderly citizens for a walk and if there is any medical requirement, they get in touch with the doctors.” Thiyagarajan Velayutham, founder and MD of India Home Health Care (IHHC) says that , home healthcare services offer many advantages including convenience, especially in a big metro where getting to and from hospital can be “difficult, time consuming and expensive”.

Rising chronic diseases are also forcing people to look at home healthcare so that they don’t need to go through frequent hospitalisation. Nurses, doctors and attendants serve the pati­ents’ needs in their own homes, covering even complex services like setting up a temoporary ICU. Home healthcare services are also more affordable than hospital stays, operating at appro­ximately 15 per cent to 30 per cent lower cost than hospital stays (see chart). “Our solutions provide 30 to 60 per cent savings on similar hospitalisation costs. Patients choose us for our deep domain expertise in specialised areas like cancer care,” says Nidhi Saxena, founder and CEO of the Mumbai-based Zoctr Health Network. Swadeep Srivastava says his company also empl­oys ex-defence personnel, who are able to handle any kind of crisis swiftly and have high integrity. “Some people are not comfortable about a stranger coming home to take care of them. If it is a defence personal, the family members are also at peace because they know that they are in reliable hands,” he adds.

Delhi-based geriatric specialist P.C. Rai, however, has a word of caution. “For bed-ridden patients this is an extremely good option. The problem starts when the nurses are not trained,” he says. Another doctor, who did not want to be named, also says that lack of trained and dedicated staff is a big issue in home healthcare. In case of acute medical emergencies, they are not very reliable, especially in the case of aged patients,” he says. It’s also not possible to transport sophisticated instruments for the use of just one person.

Then there are other issues. Homecare health service is yet to be brought under any regulatory authority, leaving patients without any mechanism to seek compensation in case anything goes wrong. The regulatory framework in the National Health Policy 2017 only covers hospitals, clinics, nursing homes and laboratories. Besides, home health care in India is not covered by most health insurance policies including the much-touted Ayushman Bharat Mission, launched recently by the Centre covering 100 million people in the first round. “Now the government’s main focus is on health insurance. Hence, it is high time for insurers to include home care to quickly grow and increase their scope of coverage. Insurers can provide it as a separate or add-on service to differentiate their product offerings,” says Delhi-based Anjan Bose, secretary general of Health Care Federation of India.

For all the drawbacks, home healthcare is growing in popularity across the country. When Mumbai-based Amar Kazi (75) injured his spine after a fall, it is his caregivers at home who nursed him back to health. “I was bed-ridden and needed help to move around. My daughters have their own family responsibilities and cannot come and live with me all the time…The attendants are trained…they take care of all my needs, starting from taking me for walks, to bathing me, giving me medicines, massaging me regularly. They are like my shadow.” For many like Kazi, there is nothing like home. And care at home.

Zoctr, a home healthcare start-up, will soon close a bigger round of Series A funding from a set of venture capital (VC) investors. The three-year old firm had recently closed a pre-Series A funding from a clutch of angel investors viz. Dubai-based NB Ventures and a host of other angel investors including Udaipur Angel Network etc. In all, the healthcare start-up has raised a little over Rs 10 crore for its business so far.
Speaking with DNA Money, Nidhi Saxena, founder and chief executive officer, Zoctr.com, said, we have already raised a pre-Series A funding earlier this month. “We are very close to raising a bigger cheque of between Rs 30 crore and Rs 40 crore. The idea is to be very focused on unit metrics. Some of our biggest competitors have a cash loss of Rs 40 to Rs 50 crore. We don’t want to go that way and be very disciplined about the way we spend our investment dollars. The plan is to create a profitable business at a very early stage,” she said adding that the new fund raise will happen in the coming weeks.

Zoctr’s business model involves billing the patient / family member on the entire value of the resource it provides. “It includes daily charges for attendants that are Rs 700 while in the case of nurses it is Rs 1,500. The intensive care unit (ICU) rates go from Rs 8,000 to Rs 10,000 per day as against hospitals that charge between Rs 40,000 and Rs 80,000 per day. So we make a 30% gross margin after excluding the direct costs. And on a net basis we are seeing close to 17% in earnings before interest, tax, depreciation and amortisation (ebitda). And the best part is that Zoctr has turned cash positive this year (around April / May) and we are not burning cash anymore. This financial year, we are expecting to achieve ebitda positive in just a little over three years since inception,” said Saxena.

The home healthcare sector in India is currently pegged at Rs 12,000 crore concentrated in eight metros and further focussed in Delhi and Mumbai markets, growing at a compounded annual growth rate (CAGR) of about 30%. Approximately Rs 6,000 crore is staffing (nurses, attendants, doctors and physiotherapists) and the balance is medical equipment, home diagnostics/ tests, home pharmacy delivery, etc.

“Right now the organised sector, between the four, five big names are doing around Rs 100 crore of business. So we are still looking at an opportunity of Rs 5,900 crore – Rs 6,000 crore minus Rs 100 crore,” said Saxena.

Saxena launched Zoctr back in 2014 after her first entrepreneurial venture Karmic Lifesciences Inc got acquired by Cliantha Research, a subsidiary of pharmaceutical major Zydus Cadila. The new venture has been growing fast dominating Mumbai and Delhi markets and is considered among the top three players.

Zoctr, which currently has four-city presence including Bangalore and Pune, competes head-on with some of the leading national players like Healthvista owned Porte Medical and HealthCare atHome (HCAH) – a joint venture between the Burman family, the promoters of Dabur and founders of HAH, UK. In addition there are a few other players like Medwell Ventures owned Nightingales Home Health Services and Care24 that have been operating in the market for some time now. “We are the only other player with a four-city operations and expanding,” she said.

The company had earlier introduced its Zoctr mobile application in the market with 3,500 to 4,000 qualified/ verified healthcare people enlisted in the database. However, after the next two-three months, the company plans to take this number to 10,000 blue-collar workers and will be the single biggest database of such people in the healthcare space in India.

“Customers can book the service just like they book a cab from the aggregator's (Uber / Ola) mobile application. Pay through digital modes of payments (PayU / Paytm) or choose to pay later. We believe even if we get 1,000 doctors on the application every month, the revenues will jump three-fold and we see our revenues easily crossing Rs 40-50 crore as compared to Rs 10 crore at present. We will soon be launching a first of its kind service in India, a remote monitoring service. The users get a wearable unit, a fitbit type cardiac watch to be precise that will look at the person’s heart rate, blood pressure rate and also EKG or ECG on a real time basis,” said Saxena adding that home chemotherapy is another area that’s been piloted and will get introduced very soon.

The company now plans to add Electronic Health Dashboard as well as Remote Monitoring features to its App. The company further plans to enter the explosive Home Cancer Care and Chemotherapy market and a large portion of this current funding round shall be utilized by the company to build deeper technology and domain expertise. The company is further creating the largest Pan India database of qualified and registered blue collar workers including Nurses, Attendants, Baby Care Nurses and Nannies on the backend of its App. The company is currently in advanced stages of discussion with a few VC funds in India and abroad for closing a larger $3-5 Mn Series A round within this year.

NB Ventures has previously invested in high growth start-ups including Healthify and Idfy and has a large portfolio of Indian companies. Garima Seth, the company’s Investment Banker and a start-up drove the current deal.

Rather than pursue senseless growth with huge cash burns, we are focusing on creating a highly differentiated niche play around Cancer Care, particularly Home Chemotherapy as well as leverage technologies such as Remote Monitoring, IoT etc. to add value to the customer.”

Home healthcare is an emerging space currently estimated at $3.76 Bn Market growing at an 18% CAGR. It is expected to touch $14.2 Bn and touch ~86 million patients by 2025.
